When Do You Need a Replacement OCI Card?

Documents Required for Lost OCI Card Replacement

  • Copy of old and new Passport
  • Copy of OCI Booklet (If you have it)
  • Photograph of applicant
  • Signature of applicant
  • Marriage Certificate (If married) OR Divorce Certificate (If divorced)
  • Proof of Employment
  • Address Proof
  • Copy of police report/crime reference number
  • Declaration from the applicant explaining the circumstances of the lost documents

 

If the applicant is a minor you will also need to provide the following:

  • Both parent's Passport copy
  • Birth Certificate
  • Parent's marriage certificate
  • If a single parent - a court order that confirms that the guardian has rights over the child
  • Consent letter from both parents, confirming their desire for their child to obtain an OCI based on Indian Origin, with a separate letter for each child. The letter should be addressed to the High Commission of India or its respective Consulates in Birmingham or Edinburgh for minor children
